Come Prepared

For your first visit please arrive fifteen minutes prior to your appointment time to allow for an efficient and timely check in. Please provide us with a photo ID, insurance card and your child’s past medical history including immunization records.

Missed & Late Arrival Appointments

Please note that a fee of $25 will be charged in cases of missed appointments or appointments that are not canceled at least 24 hours in advance. This policy is outlined in our new patient registration form.

In the event of a late arrival, we reserve the right to reschedule your appointment and/or charge $25. 15 minutes late for established patients, and 10 minutes late for new patients, is considered a late arrival for any visit type.
